What is Principal Care Management (PCM)?
Principal Care Management (PCM) is a healthcare service designed to provide comprehensive, patient-centered care for individuals with a single chronic condition. It aims to enhance the quality of care and improve health outcomes through continuous and coordinated management.
Unlike Chronic Care Management (CCM), which requires two or more conditions for Medicare patients to be eligible, PCM allows for providers to render services when the patient only has one chronic condition.
Typical conditions include diabetes, hypertension, chronic obstructive pulmonary disease (COPD) or heart disease, chronic kidney disease (CKD), end stage renal disease (ESRD) on dialysis, afib, arthritis, and more.
