What is Chronic Care Management (CCM)?
CCM offers Medicare beneficiaries a specialized healthcare expert who assists in overseeing chronic conditions. This involves collaborating with your physician, specialists, pharmacy, testing/diagnostic facilities, and local community resources.
What type of services does CCM provide for me?
Chronic Care Management provides the following services to eligible Medicare beneficiaries:
- At least 20 minutes a month of chronic care management services.
- Personalized assistance from a dedicated healthcare professional who will work with you to create your care plan.
- Coordination of care between your pharmacy, specialists, testing centers, hospitals, and more.
- Phone check-ins between visits to keep you on track.
- 24/7 emergency access to healthcare professionals.
- Secure electronic access to your care team.
- Online electronic access to your personalized care plan.
- Timely electronic information sharing with specialists, including care plans.
- Coordination with specialists, hospitals, and other medical facilities.
- Coordination with other services (such as home care, durable medical equipment, and community resources).
Who is eligible for the service?
All Medicare patients (with or without a secondary payer) with two or more chronic conditions are eligible.
